2: Settings:

you will something like shown in below image. Now just follow below steps.

a) Check the box which you want to show in your contact page form. Like I have checked name and email. b) In this step shown in image as step 2. Check this box if you want to make this as required. Use of this box is to fill that information is essential

c) See the 3^rd step in image. It will show other options to have in your contact form. After clicking this you will get options. See image below. Check the option you want.

3: advanced settings:

In option two you will see the advance settings. Here you can change your color as required for your blog or website.

4: Targeted Email address:

In this you have to add your email address where you want to get your queries.

5: Now you will see the preview as shown below

6: Now just click on the create formula button

7: After this you will get the code. Now you just need to add it in your blog or website.

2: Now just go to html editor in that and just paste the code as shown below. You can also add your link as I did. See the arrow in below image.
